Teen Volunteer Application Form: Teen Advisory Board and/ or Page Volunteer



By filling out this form, you are expressing your interest in a volunteer opportunity at Southwick Public Library. The library will contact you in late October or early November to follow up with you about the positions and next steps.  This application is for either the Page Volunteer, or the Teen Advisory Board Volunteer. If you feel you can commit to both, you are welcome to participate in both opportunities.

Page Volunteers- help with shelving, organizing, preparing book and bulletin board displays for the young adult area, and other activities as needed. This opportunity requires that volunteers commit to an hour a week at a mutually agreed upon time.  There will be an interview session for new applicants in early Monday November 7th at 3:00.  The training session will take place November 14 at 3:00. 

Teen Advisory Board– Create and plan teen activities and events, as well as      suggesting materials for the teen collection. Commit to meeting the second Tuesday each month from 2:30-4:00 for 4 sessions. Our first meeting will be November 12.
